We really appreciate you taking the time to give us feedback.

 

We have embedded quality learning points as part of our quality and improvement process and are already regularly reporting on aspects of the service that went well, or could have gone better. 

We work on the understanding that none of us have come to work to try and do a bad job, the team are working hard to deliver a responsive, accessible and quality healthcare service. 

We cannot deliver a patient focused service without hearing from you and would really like to hear from you about what went well, and what could have been better?

This form only takes a few seconds to complete and will reach the manager email account directly. We will respond with thanks for your feedback and will add your (anonymised) feedback to one our Team /  Practice Meetings for reflection and discussion.
